There are quite a few different types of acne, depending on how or why the acne arose. Some of the types of acne are: acne conglobata (chronic boils); acne fulminans (an extreme form of conglobata); acne cosmetica (caused by cosmetics); acne keloidalis nuchae (from shaving); acne medicamentosa (caused by starting or stopping a medication); acne rosacea (redness on the face); baby acne; hormonal acne; cloracne and the common variety: acne vulgaris (better known as ‘puberty spots’). We will take a closer look at acne vulgaris.

Acne vulgaris may be defined as: ‘an inflammatory disease of the skin, caused by changes in the pilosebaceous units (skin structures consisting of a hair follicle and its associated sebaceous gland). Acne lesions are commonly referred to as pimples, spots or zits’ (from Wikipedia).

Acne is most often found in Western teens, although it does occur in every country in the world, so there could be a genetic predisposition to it. It is possible that it could be an exceptional reaction to fairly normal levels of testosterone. For most sufferers, the condition lasts only until adulthood, probably only a few years or at most ten. For others, however, it can be a life-long recurring problem. It most often affects the face, upper-chest, upper-arms and back. However, an occasional spot is not the same as acne.

Acne vulgaris manifests itself in different many ways, including: whiteheads, resulting from pores that are completely blocked, trapping sebum (oil), bacteria, and dead skin cells, causing a white appearance on the top; blackheads, resulting from pores which are only partially blocked, allowing some of the trapped sebum, bacteria, and dead skin cells to slowly drain to the surface (the black colour is not caused by dirt, but is a reaction of the skin’s own pigment, called melanin, with the oxygen in the air); papules, which are inflamed, red, tender bumps with no head and pustules, which are like whiteheads, but are inflamed, and look like red circles with a white or yellow centre.

Whiteheads do not usually last for a long time; blackheads can last a long time and pustules are what people usually refer to as spots or zits. Severe acne vulgaris is characterized by nodules and cysts. A ‘nodule’ is a rather larger and much more painful sort of pustule and may sometimes last for months. Nodules are large, hard bumps just under the skin. They frequently result in scarring and should never for any reason be squeezed, since this could make them last for months longer.

A ‘cyst’ may appear like a nodule, but it is full of pus and has been defined as having a diameter of at least 5mm and, again, can leave scars and cause irritation. Squeezing an acne cyst can cause a deeper infection and more painful inflammation which may last very much longer than if it had not been squeezed. Skin experts have methods of lessening the swelling and preventing scarring with both nodules and cysts. It is a myth that acne sufferers are not careful about washing.

Indeed, it is a little-known fact that, excessive washing can irritate acne. There are many, many useless ‘treatments’ in the shops and many, many old wives’ remedies. However, any honest skin expert would tell you that there is no sure-fire remedy for acne and that the patient should follow a strict regimen of cleansing until the acne just ‘disappears’ of its own accord.

Acne is a disease of the pilosebaceous models, also referred to as PSUs. PSUs are located in nearly all locations of the body. They include a gland that is related to a canal that is referred to as a follicle; the follicle comprises a fine hair. The largest number of PSUs are situated on the face, upper back and chest. Sebum, an oily substance secreted through the glands, finds its way to the floor of the pores and skin by the skin pores or the opening of the follicles.

The follicle is lined with cells often known as keratinocytes. A plug may be created as a result of the hair, sebum, and keratinocytes which can be situated in the slender follicle, and that is an early sign of spots. The stop stops sebum from accessing the skin’s surface via a pore. The combination of oil and cells allows bacteria Propionibacterium acnes (P. acnes) that sometimes exist on the skin to form inside the plugged follicles. White blood cells are interested in the chemical compounds and enzymes created by these bacteria, and the white blood cells cause inflammation. (Irritation is a typical reaction of tissues to illness or harm and has four attribute symptoms: enlargement, redness, heat, and discomfort.) As the wall of the plugged follicle deteriorates, it drops its contents into the adjacent pores and skin – sebum, shed pores and skin cells, and bacteria – resulting within the formation of lesions or pimples.

Wash often using a soap that is not too severe and that is not over-perfumed; but do not wash so frequently that your skin loses its natural oils and becomes dry. Rub on moisturizer to your body once a day after a shower or bath and give the moisturizer time to soak in before you towel yourself dry.

If you go out in cruel weather like wind or cold, moisturize before you go out and cover up as much of your skin as you can. Shower and moisturize when you get back. It is better to have a small pot of expensive but very beneficial cream, in particular for your face, than a large container of something that is little better than mayonnaise.

You must take care not to get any lotion or cream directly onto the lips or any unaffected area of the skin. If necessary you could mask some of the spots with a spot treatment preparation. Try to remember to clean the affected areas every morning with the tepid water and cleanser treatment.

Directly after your affected area has been cleansed you can add the Benzoyl peroxide to the area that is affected. Within a week the results should start to show.

If after a week your symptoms are the same and the spots are still there then you may want to consult a doctor about them. It is quite possible that it may in fact be perioral dermatitis. The treatment for this is different to acne but the symptoms are the same with spots appearing around the lips, chin or nostril areas. You can treat perioral dermatitis with topical or oral medications.
